age and the head-up tilt test outcome in syncope patients

چکیده
conclusions hemodynamic response to the hutt was associated with age. cardioinhibitory response became less frequent with age due to exaggerated vagal activity in the younger patients as compared with the older subjects. background the head-up tilt test (hutt) is a useful diagnostic tool for syncope. objectives we sought to investigate the outcome of the hutt in syncope patients and identify the relationship between age and different hemodynamic outcomes. patients and methods in this cross-sectional study, we prospectively enrolled consecutive patients who presented with syncope and underwent the hutt with a clinical suspicion of neurocardiogenic syncope after the exclusion of orthostatic hypotension cases. the hutt consisted of consecutive passive and active phases. in the passive phase, the patients were tilted at 70 degrees for 20 minutes; and if negative, the test was repeated with 400 micrograms of sublingual nitroglycerin for another 20 minutes. positive responses were classified according to the classification of the vasovagal syncope international study (vasis) and compared for age and gender. results a total of 498 patients were enrolled (age = 44.93 ± 18.77 years; male = 271 [54.4%]). overall, 291 (58.4%) patients had a positive hutt, while 256 (88.5%) patients had a positive result during the active phase. the test results were as follows: 107 (36.7%) mixed type (vasis i), 103 (35.3%) cardioinhibitory (vasis iia = 44 [15.1%]; vasis iib = 59 [20.2%]), and 80 (27.4%) vasodepressive (vasis iii). there was no relationship between gender and syncope type. the trend of the hutt result significantly changed with age, and the rate of cardioinhibitory syncope decreased after middle ages (p value for trend = 0.02).

Age and the Head-Up Tilt Test Outcome in Syncope Patients
BACKGROUND The head-up tilt test (HUTT) is a useful diagnostic tool for syncope. OBJECTIVES We sought to investigate the outcome of the HUTT in syncope patients and identify the relationship between age and different hemodynamic outcomes. PATIENTS AND METHODS In this cross-sectional study, we prospectively enrolled consecutive patients who presented with syncope and underwent the HUTT with ...
متن کاملDiagnostic value of the head-up tilt test and the R-test in patients with syncope
The diagnostic value of the head-up tilt test (HUTT) in discovering vasovagal syndrome depends on the pre-test probability. An accurate anamnesis and clinical examination screens the patients indicated for the HUTT. In patients with unexplained syncope, the R-test is an alternative procedure to discover its cause. In our study, we evaluated the diagnostic significance of the HUTT in a group of ...
متن کاملHead-up tilt test in complicated neurocardiogenic syncope in children.
To confirm the usefulness of head-up tilt test (HUT) in neurocardiogenic syncope (NCS) with complicating clinical features, retrospective analysis were done on 12 selected children. The age at onset was 12.7 +/- 1.9 (mean +/- SD) years. Associated clinical features were postoperative congenital heart disease (PO CHD) in 3, coexistent arrhythmia in 8 (persistent ventricular arrhythmia during exe...
متن کاملHead-up tilt test for recurrent syncope in pilots.
Syncope is defined as a sudden temporary loss of consciousness and postural tone that is associated with spontaneous recovery. Vasovagal or neurocardiogenic syncope is a common and usually benign cause of syncope. The mechanism may be cardioinhibitory, vasodepressor, or both. Diagnosis is usually made by a typical patient history with a definite trigger. Although vasovagal syncope is considered...
متن کاملSeizure-Like Activities during Head-Up Tilt Test-Induced Syncope
PURPOSE Some patients with neurally mediated reflex syncope may be misdiagnosed as epilepsy because myoclonic jerky movements are observed during syncope. The seizure-like activities during the head-up tilt test (HUT) have been rarely reported. The purpose of this study was to assess the characteristics of these seizure-like activities and evaluate whether there are differences in the clinical ...
متن کاملPredictors of response to the head-up tilt test in patients with unexplained syncope or presyncope.
BACKGROUND Unexplained syncope is a relatively frequent symptom, mostly caused by a neurocardiogenic reaction. The purpose of this study was to determine predictors of response to head-up tilt testing (HUTT) in patients with unexplained syncope or presyncope. METHODS HUTT was done in 640 consecutive patients with unexplained syncope or presyncope (393 men, mean age 45+/-19 years) after initia...
